Health Form

Participating in Pack and Den activities (i.e., camping and outings) requires completion of Annual Health and Medical Form for each family member attending. The Pack will generally gather these for each scout at the begining of the school year.  The form can be accessed through the link below.

Shooting Sports Permission Form

For outings where we have shooting sports - primarily BB guns and archery for cub scouts - the Shooting Sports Authorization Form must be filled out as well to participate.  The form can be accessed through the link below.
